Once you have read the overview/pre-requisites please click above link for the application form. Upon completion of the application form, pay particular attention to the preamble of the application regarding how the application is to be submitted. Be advised that applications not accompanied by required documentation, will result in a rejected application.
Registration Fees
DO NOT PAY registration fees until you are notified that your application has been approved. You will be advised and directed to a specified page to make payment via PayPal (Visa / MasterCard accepted) when the approval process has been completed.
